CUSTOMS DECLARATION FORM

A customs declaration must be sent with the samples. There are a few different forms depending on where the samples are from and if they require quarantine.

International samples:

Customs Declaration - PDF Version - Word Version

Samples from within Australia:

Customs Declaration - samples from greater depth than 2m - do not require quarantine - PDF Version - Word Version

Customs Declaration - have been heat treated to specifications - do not require quarantine - PDF Version - Word Version
